Please write your name, address and telephone number on the reverse side of the cheque. Application forms for the monthly pass must reach Bus-Plus Services Pte Ltd by the 15th of the preceeding month that you are applying for.

There will be no refund for public holidays and any unusual portion of the booked period. Kindly note that there will be no stops along the route, except at designated alighting points.
